Patient registration

If you live within the Practice boundary, you can apply to register with us.

The practice agrees to accept all patients, within the catchment area, requesting to join the list. We do not discriminate on the grounds of age, race, gender, sexual orientation, religion or medical condition.

Due to the COVID-19 restrictions, we cannot accept registration forms at the reception, please mail them to us. If you have any questions, or are on any medication, please call the reception on 0131 333 0600.

You can download and hand-in the registration forms from our website, details below. Medical treatment is available from the date of registration. If you are on medication please let reception know when you return your form and they will arrange an appropriate appointment with a doctor.

Our patient registration is completed in three main steps.

Prerequisites

Before you begin, you need to have or know the following.

  • You must live within the practice boundary (details below)
  • You will be required to supply proof of address
  • You must be staying in the area for more than three months

If you will not be in the area for more than three months, please contact the Practice so we can make alternative arrangements for a temporary registration.

Step one of three: Check the Practice boundary

Patients are required to live within the Practice boundary. We have provided a map with the boundary drawn below, if you need assistance determining if your home is within the boundary, please contact the Practice.

Step two of three: Complete your health questionnaire

Patients are required to complete a health history questionnaire. This is a paper form, and can be downloaded, completed, then sent to us via the website. You can also collect a pre-printed form from the Practice, and send or hand it in at a later date.

Step three of three: Submit your registration details

Your registration details are submitted online using our web form. It will take approximately 10-15 minutes to complete.

What happens after I register?

You will be offered a new patient medical check. If you choose to accept this, it will be carried out by a member of our nursing team. If you are on any repeat medication you will require a GP consultation.

When you register you will be added to the practice list and not to the list of an individual doctor and can be seen by any of the doctors. It may not always be possible to see the doctor of your choice on a particular day or time.
